Severity of symptoms

Is there anywhere that says what they actually mean by "no symptoms" "intermittent symptoms" "mild/moderate daily symptoms" " persistent, severe symptoms" ?

Are they even a defined term? Or based on whoever is assessing for that day and how they are feeling?
