For the application, is it mandatory that we give three different choices for trades? I've put down medical, and infantry, but there's not much else that I'm interested in.

Its fine, I know people that just put down one choice.

As long as you have one choice they really don't care. Just be ready to wait a bit if the choice(s) you have aren't open at the time you apply. But from you the ones you listed you shouldn't have hard time with getting in as long as you pass everything.

Alright that's cool thanks...but am I supposed to write the course number down? Or can I just put "Medic" or "Infantry"?

You might as well throw th 3 digit moc number down. Of course they will know what you mean anyways.
