must be reported and documented.
Documentation
The commanding officer (CO) of a CF member shall ensure that a permanent record of the relevant circumstances pertaining to an injury, or exposure or suspected exposure to a toxic substance, is kept on the CF member's permanent personal file. The record shall include all relevant information including the way in which the circumstances relate to the military mission or task.
Process
Recording
Incidents of injuries as well as exposure or suspected exposure to toxic substances
shall be recorded on form CF 98, Report on Injuries or Exposure to Toxic Material.
Temporary Duty
When a CF member on temporary duty is injured, or exposed or suspected to have been exposed to a toxic substance, the CO of the host unit
shall ensure that form CF 98 is completed and distributed, and that an information copy is sent to the parent unit of the CF member.
