I wonder if someone cut the tail too short when it was inserted - usually they're left long enough that the woman can pull it out themselves if they so desire or at least be seen on a direct visual internal exam. Also, often people getting IVF have some sort of imaging like a hysterosalpingogram (dye shot of the uterus and Fallopian tubes) just to make sure there's not an anatomic issue...not like a little piece of metal or plastic designed to be seen by XRay wouldn't show up on that.
