Gave DND 2765 last time when they asked for medical information related to ADHD-ADD. And now I have to complete a new form ? Does anyone know what this one is about as I'm yet to receive it by mail...

The form is titled "Psychiatric - Information for Recruitment". It would be for info different from the ADHD form.

Can a generalist sign the form or does it have to be a specialist ?

It is a form requiring psychiatric information. Therefore, it will only be accepted if it filled out by a psychiatrist
