The city of Ottawa is instructive in their Light Rail system; fully contracted out, the stations are already is disrepair, the trains although getting better have had many breakdowns, some shutting the system for weeks.
The challenge is that procurements were made based on sizing distributions from the 70s. So there are thousands of original CADPAT items that have never been issued in smaller sizes, and no stock in common sizes.

Not testing, not training, cutting funding, no fallback plan, firing all the experiences staff and relocating the function to NB just before launching the new system... So many choices for "why was it a cluster fuck of a disaster of a failure".
